Documents pertaining to the declaration of a birth

Here are the documents pertaining to the declaration of a birth:

• The Attestation of Birth (DEC-2). This form is attached to the Declaration of Live Birth. It is completed by the birthing physician or the midwife.
• The Declaration of Birth (DEC-1). This form exists in electronic and paper versions. It is completed by the children’s parents.
• The Application for Simplified Access to Birth-related Government Programs and Services (DEC-3). It is attached to the Declaration of Birth and offers the parents the possibility of notifying several departments and agencies of their child’s birth in a single step.

We must receive the copies of the Attestation of Birth and Declaration of Birth forms intended for us to enter the birth in the Québec Register of Civil Status.
