Attestation of Birth
The birthing physician or the midwife who assists the mother during childbirth completes the Attestation of Birth.
The staff of the health institution or the birth centre provides you with a copy of this document and sends the original to us.
Important
When you complete the Declaration of Birth, you will need the Facility Code, entered in box 2 of your copy of the Attestation of Birth and the attestation number, entered on the right at the bottom of the page.
