Documents related to declaring a death

The following forms are related to declaring a death:

  • Attestation of Death (DEC-101)
  • Declaration of Death (DEC-100)
  • Application for the simplified forwarding of information relative to the death (DEC-102). This application is attached to the Declaration of Death and allows the declarant to notify several departments and agencies of the death in one step.

The Directeur de l'état civil must receive the copies of the forms Attestation of Death and Declaration of Death in order to enter the death in the Québec register of civil status.
