You may choose to declare your child's birth by completing the Declaration of Birth.
The form may be provided on request, after childbirth, by the staff of the health institution or the birth centre. You may also contact us by phone to obtain one.
It is best to give the completed and signed Declaration to the staff of the health institution or birth centre before you leave. The staff will send it to us along with the Attestation of Birth.
If you choose to complete the form at home, it is your responsibility to send it to us, once it is completed and signed, within 30 days of the child’s birth, at the following address:

Important
If you choose to declare your child’s birth on a paper form, you must not declare it again by using our Electronic Declaration of Birth online service. It is important to use only one means to declare your child’s birth, because receiving two Declarations for the same child will lead to additional processing delays.
